Abstract: Objective: to describe the clinical-epidemiological profile of pregnant women hospitalized in Pernambuco with SARS caused by COVID-19 in 2020, relating to the main factors associated with death. Methods: retrospective cohort. The analysis is based on public data from the Influenza Epidemiological Surveillance Information System (SIVEP-flu), a database referring to severe acute respiratory infections, including COVID-19. Result: a total of 257 pregnant women were analyzed. Most aged 20-30 years (43.6%), in the 3rd trimester of pregnancy (79.1%), living in the urban area (95%) and brown (81.5%). Of the sample, 24 (10.8%) required ICU admission, 18 (7%) required invasive ventilatory support, 32 (12.5%) required non-invasive ventilatory support, and 19 (7.8%) died. Maternal mortality had a statistically relevant relationship with older age, the third gestational trimester and the presence of dyspnea, respiratory distress and O2 saturation < 95%. Conclusion: The population of pregnant women in Pernambuco follows the Brazilian trend of higher mortality rate compared to world statistics. Older age, 3rd gestational trimester and presence of dyspnea, respiratory distress and O2 saturation <95% were the main findings related to increased risk of complications and death. New studies must be conducted to expand and enhance the relevance of the data found in this research
